Medication

Please give medications at home whenever possible. However, if medication is needed during the school day, the following is necessary. Please refer to the School Medication Policy or contact the nurse's office if you have additional questions.

PRESCRIPTION MEDICATION (EpiPens and Inhalers included)

1. Parent/Guardian Prescription form must be filled out.
2. Medication must be in a properly labeled prescription bottle.
3. Physician Prescription form must be filled out and returned.

In many cases the health aide will fax the form to the proper physician.

OVER-THE-COUNTER MEDICATION

In the Hortonville Area School District we supply ibuprofen or acetaminophen when needed. Anything else must be provided by the student. Before providing the medication OTC Medication form must be filled out and returned to the health room. In the high school, students are allowed to carry their own over-the-counter medications with parental permission.
