INTRA-DISTRICT SCHOOL REQUEST

This form is to be completed for the following types of requests:

1. If you want your child to attend one of the district schools not in your current attendance zone. 
2. If you are planning to move into the district and would like your child to attend the school in your attendance zone.
3. If you are planning to move into the district and would like your child to attend a school not in your attendance zone.
