Medication Forms
In compliance with Wisconsin State Law, the Hortonville Area School District has adopted a policy for the administration of medications. For school personnel to administer medication safely and efficiently, parent/guardians must comply with district policy. Whenever possible, please give your children their medications at home. We encourage you to work with the prescribing provider to determine if the medication can be scheduled outside of school hours. For all medication that will be administered at school, the parent/guardian, not student, needs to bring the medication to school. Parents must assure that a backup rescue medication is available to their child after school hours and when traveling to/from and during school-sponsored events.

*Per HASD medication policy, non FDA-approved medications cannot be administered without a physician’s consent.
Emergency Action Plan
Providing a safe learning environment for our students is key. In order to accomplish this, we need your help to provide our staff with medical information that would best equip our staff to respond to meet your child's needs. We encourage families to discuss your child's health information and to create a health plan that would best meet your child's needs. If your child has a health condition that is not listed above, the school district will accept health plans that are created by your child's health care provider.

Dietary Needs
If your child has a special dietary need that you would like accommodated for school lunch, please visit the Wisconsin Department of Public Instruction's School Nutrition page and fill out the "Medical Statement." Once completed, please email to your school nurse.
